    Tournament Registration - 2015

Thank you for registering for the 30th annual Hopewell Valley Soccer Association Memorial Day Weekend Tournament.

The schedules will be posted in mid-May.  

Please check schedule often as schedule is SUBJECT TO CHANGE.
 

E-REGISTRATION / CHECK-IN OPTION

 
Teams are encouraged to register electronically.  By using e-registration you are certifying that you have signed and current medical release forms for each rostered and guest player on your team.

E-registration is open now and ends at 11pm on Thursday May 21st.  Teams that do not register electronically will be required to register in person at least one hour before your first game on Saturday May 23rd  

Instructions for e-registration are:
 

  1. Scan the required document(s) and e-mail them to:
  2. In the Subject line for the e-mail, please label "Registration [age group/team name]."  For example: “Registration U14G Hopewell Dust Devils.”  We request that Teams send ONE e-mail with ALL requested documents.  Please send them in PDF or JPEG.  

 3.  Documents required:       

  • Current approved, stamped roster.  Ineligible players or those players who will not play are to be completely blacked out or crossed off.  Note that Guest players count towards the maximum number of players permitted.
  • Guest player form (<-- click on link to download) listing the guest players (any club/tournament guest player form will be acceptable).
  • For all players (including guest players), a copy (front and back) of their current player card.
  • For all players (including guest players), a copy of their Signed Medical Release form.

All teams are to have available at each game their approved, stamped roster, all player and coach’s cards, all medical release forms (signed), and as applicable Guest Player Form and Permission to Travel Form.  Note that Permission to Travel Form is only required for teams outside of Region 1.

The same documentation will be required for teams that choose to register in person.

A confirmation email of registration/check-in will be provided.  If any additional information is required teams will be notified before Friday May 22nd.
 
If a team that e-registers is not notified that they are approved for play, then that team must register in person before their first game on Saturday May 23rd.
